Define the truck, attachment and load
Record the truck manufacturer, model, serial, mast, current forks, plate information and existing attachments. Define the proposed attachment by manufacturer, model, serial, mass, center-of-gravity data, mounting class, hydraulic or electrical needs and load-handling function.
OSHA requires prior written manufacturer approval for modifications or additions affecting capacity or safe operation and requires plates, tags or decals to be changed accordingly. HSE guidance also places derating and capacity information with competent assessment. [1] [2]

Procure the attachment with an approval package
Issue the RFQ with complete truck and load data. Require the proposal to identify the attachment, supplied mounting and service components, approval route, capacity-information deliverable, installation responsibility and operator or maintenance documentation.
Review visibility, fork or load support, hydraulic flow and pressure, electrical interface, hose routing, guard clearance and parking or transport envelope. These fields support engineering review; the approving authority defines the final configuration and capacity.

Commission and control the configured truck
Before release, verify the physical attachment identity, secure mounting, service connections, stops or locks, visible labels and current capacity information. Complete the responsible acceptance process and train operators for the new load-handling and visibility characteristics.
Add the attachment to the truck asset record and inspection checklist. Prevent removal, transfer or replacement outside change control. Reopen approval when the attachment, forks, mast, load, truck or operating method changes.
